Fill in the information about who you are and what package you would like to select.
Payment can be made by one of three methods:
- E-Transfer – Follow the instructions to use Interac(R) e-Transfer to send money to our Finance Manger. Email the form to the email address provided.
- Credit Card – Email the form to us and you will receive an invoice by email. You will be able to pay the invoice online via Square.com. We never see or handle your credit card.
- Cheque – Mail the completed form and the cheque to the address provided.
Save or print the completed form and for electronic payments email the form to the email address provided.
For a payment by cheque, print the form and mail it ot the address provided.
